After filling up this registration form, clicking the DOWNLOAD button will download the PDF file to your computer or smartphone. Thus, clicking the DOWNLOAD button WILL NOT automatically REGISTER the student. To be registered, you will have to send the downloaded PDF file to SLCS using the Enrollment Registration page which can also be accessed on the top menu of this site. Once the said PDF file is received by SLCS, you will receive an email for confirmation and further instructions on how to enroll.
