Online Release of Requested Documents

For the safety of everyone, requested documents (ex: certificate of enrollment, certificate of good moral for grade 6 students) will be released via email.
Please follow the steps below.
1. Make a request via e-mail.
Please follow the format below.
(Date)
Requesting a copy of (document needed) SY _____ for student (please type student’s full name), (Level and Section)
Full name of the parent/email address

Note: For proper documentation, all requests that will be entertained are via EMAIL only (no text or messenger requests) since the document will be sent ONLY in your indicated email address.

  1. Email the request to daisygonzalesslcs@gmail.com
    This is the only email that will process the request for documents. Kindly give us 1-2 days to process your request.
  2. The document will be sent to you by kidsachieve@gmail.com. This is the ONLY email that will be used in sending requested documents.
